Senior Social Worker - Home Based Primary Care Program (HBPC)
The major duties and responsibilities include, but are not limited to:
Performs specialized assessment and treatment of complex physical and/or mental illness, beginning with an assessment of the veteran in his/her residence, in collaboration with the interdisciplinary treatment team, family members and significant others, primary caregivers, and other community care professionals. Provide psychosocial support to the veteran and identify the need for transitional levels of care when appropriate. Comprehensive psychosocial assessment completed by the HPBC senior social worker, that includes interdisciplinary consultation, the incumbent is responsible for developing achievable treatment goals with the veteran, family, caregivers, and other care professionals associated with the veteran. Works independently and autonomously in the community and provides comprehensive assessments and interventions in the home and other places of residence that include, but are not limited to, residential care facilities (RCF), public housing, senior housing, and single residential occupancy hotels (SRO). Providing counseling to veterans and families about illness and loss; crisis intervention; advance directive education; eviction prevention, coordination of linkages to other VA or community service providers/agencies; end of life issues and support, and consultation to interdisciplinary team members. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Education:
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure:
Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. Exception. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies up to the full performance level (GS-11). For grade levels above the full performance level, the candidate must be licensed or certified. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations:
In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. GS-12 Senior Social Worker. In addition to the basic requirements for employment, Senior Social Workers must have experience that demonstrates possession of advanced practice skills and judgment. At least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. In addition, the candidate must demonstrate the professional KSAs below:
(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. References:

The physical demands of this position involve active listening, reading, observing and evaluating verbal and nonverbal behavior, walking, speaking, writing reports, writing policies and procedures, writing clinical notes, using office equipment such as computers, telephones, fax machines, photocopy machines, and driving throughout the VISN. The incumbent must possess a valid driver's license.
Estimated Salary: $20 to $28 per hour based on qualifications.
